[Concepts of Soft Tissue Layers & Spaces]

face lifting에 대해서 알아보기 전에 안면의 해부학적인 이해는 필수적이다.

이와같은 복잡한 anatomy를 이해하기 앞서서 안면의 기본 구조를 이해하는 것이 도움이 된다.

안면과 scalp는 몇 겹의 layer로 구성되어 있는데, 이는 구체적으로 다섯 가지 기본 layer로 구성되어 있다.

(1) Skin

(2) Subcutaneous layer

(3) Musculoaponeurotic layer (SMAS)

(4) Loose areolar tissue (ie, spaces and retaining ligaments)

(5) Fixed periosteum and deep fascia 

이중 우리가 가장 주목하여야 할 부분은 '(3) Musculoaponeurotic layer'이다.

‘Musculoaponeurotic layer (layer 3)’는‘ subQ layer (layer 2)’의 retinacular cutis fibers로 ‘skin (layer 1)’까지 붙어있다. Musculoaponeurotic layer는 upper face에서 scalp로 넘어가면서 ‘galea' 와 flat muscle인 ’occipitofrontalis‘로 합쳐진다.

또한 목으로 내려가면서는 platysma muscle 로 합쳐진다.

(1), (2), (3) 이 세 가지 layer가 ‘functional unit’를 형성하는데, flap surgery 등에서 하나의 단위로 박리되고 이식된다. 보통 ‘(3) Musculoaponeurotic layer’를 ‘Superficial MusculoAponeurotic System (SMAS)'라고 표현한다. SMAS는 fascia와 여러 가지 facial intrinsic muscle 들을 포함하고 있다.

Functional unit (1+2+3)가 periosteum 위에서 움직일 수 있는 것은, '(4) loose areolar tissue'가 있기 때문이다. 이 네 번째 layer는 두 가지 상반된 중요한 기능이 있다. 첫째는 facial bone에 soft tissue가 잘 붙어 있을 수 있게 한다. 둘째는 bone위에서 soft tissue 들 움직일 수 있는, 즉 gliding movement가 가능하다. 피부 노화로 aging laxity가 심해지면 이 gliding plane의 공간이 넓어지게 되는 임상적인 의미가 있다. Facial nerve 들과 vessel 들이 (4)번 layer 밖으로 다니는데, layer내를 통과하지는 않는다.
